I understand that gymnastics is a sport with inherit dangers, despite due considerations for safety issues, I give permission for my child to participate. I further understand that from time to time, my child may be transported to external events as a representative of the club – under these circumstances people travel at their own risk (please type your name in the area provided below).

I give permission for my child to be photographed/videoed for use within our notice boards, on our website and private Facebook page, as well as in the media (please state your childs full name in the area provided below)

I have been made aware of the statements in the members pack along with the risks associated with concessions made on the code of conduct and dress including piercings and jewellery. I accept full responsibility for my child to participate and indemnify the coach, organiser or BG official against any responsibility as a result of their attire (please state your full name and your childs full .
